The information in this form is collected by the Cowboys Rugby League Football Limited (the Cowboys) and will be handled in accordance with our privacy policy, which is available on our Privacy Policy page. 

Our privacy policy sets out our contact details, and information about how you can access and seek correction of your personal information that is held by us, and how to make a complaint.

We are collecting the information you provide through this form to help us allocate a ‘best-equivalent’ seat and access at the new stadium which meets your needs.

We only collect your sensitive information (including information about your health) with your consent, so it is your choice whether or not you provide this information to us.

If you choose not to provide us with this information, we will not be able to take into account any medical needs you have when allocating you your 'best-equivalent' seat.

The Cowboys may also disclose your information through the course of this collection process to other service providers, specifically the SurveyMonkey platform for hosting and data storage. The Cowboys require this third party to keep your contact details and personal information confidential and only use them to provide services to the Cowboys. 

SurveyMonkey identifies you, in responding through this form, as a Respondent and has provisions for the safety of your data in their Privacy Policy.

As a Respondent, where you have responded to a survey, form, questionnaire or application sent to you by a Creator (the Cowboys), using a SurveyMonkey service, you will need to reach out directly to the Cowboys to discuss managing, deleting, accessing, restricting access to or otherwise withdrawing consent for use of, the information which you provided in your responses.

SurveyMonkey does not control your response data and, accordingly, is not in a position to directly handle these requests in relation to that data. All response data at an individual level is controlled by the Cowboys. Where response data is analysed by SurveyMonkey, this is only done so once the anonymity of individual respondents is ensured (by aggregating and anonymising the data). Personal data for Respondents outside the United States is processed by SurveyMonkey Europe UC.

We will not disclose this information to any other person or entity without your consent.

The information you provide will be used exclusively for the purpose of allocating your 'best-equivalent' seat and the supporting documentation you provide will be deleted or destroyed from both Cowboys' records and SurveyMonkey data hosting following the 'best-equivalent' seating allocation process.
